Health & Hunger Pilots/Projects

Pilot for West Orange Healthcare District: 3/17 through 3/20; Extended through 5/21
3 year pilot to evaluate the effect of access to healthy, diabetic-appropriate food, along with periodic nutrition education, in improving clinical and behavioral outcomes for low-income patients referred from the Winter Garden Community Health Center.

Pilot for Orlando Health: 8/17 through 7/18 (now extended through 12/20)
1 year pilot to evaluate the effect of access to healthy nonperishable foods and fresh produce for very low income patients discharged from hospital and being case managed by a hospital outreach team.

Pilot for Orlando Health: 2/20 through 2/21
1 year pilot that will correspond with the healthy food access pilot; the goal is to provide transportation to the referred program participants who lack transportation to and from the participating pantries.Healthy Food Boxes funded by both Aetna and Adventist Health Services Population Health Services for distribution through SHFB Healthy Pantry Network.

Pilot for Advent Health Community Care: 9/18 through 2/20
2 year pilot to evaluate the effect of access to healthy, diabetic-appropriate food, along with periodic nutrition education, in improving clinical and behavioral outcomes for food insecure patients with multiple diagnoses, including at least one chronic, diet-related disease referred from Florida Hospital Apopka.

Pilot for Advent Health Community Health Impact Council: 1/19-12/21
3 year pilot to develop approximately 10 Food Bank partner agencies to expand the Central Florida Healthy Pantry Network, identify and develop referral protocols within hospital system, and provide diabetes/hypertensive appropriate foods and nutrition education to referred low income, food insecure patients at five Advent Health Central Florida campuses.

Pilot for Orlando Magic Youth Foundation: 2/19-1/22
3 year pilot that will evaluate the effect of increased access to healthy, diabetic-appropriate food, along with periodic nutrition education and dietetic counseling for children and their caregivers referred from Grace Medical Home.
