The Medically Tailored Meals program aims to help people experiencing food insecurity and managing at least one severe illness improve their health outcomes through food. Food insecurity not only increases the risk of someone developing health issues but can also worsen the complex challenges of managing a chronic illness.
At Mercy Kitchen, Second Harvest Food Bank and its volunteers can prepare nutritionally-tailored meals specific to an individual patient's needs and deliver them directly to their homes.
A pilot program funded by the DaVita Giving Foundation allowed Second Harvest's work around Medically Tailored Meals to take shape, and the number of neighbors served is already growing. Local healthcare providers can now refer eligible patients with chronic kidney disease to the food bank for enrollment.
Second Harvest plans to expand MTM by working with a patient's medical team to help follow a prescribed diet, including when more than one chronic illness coincides.
